Family physicians spend an average of 2.7 hours per day on after-hours EHR work and 12–13 hours per week on prior authorization alone. A rural, bilingual, solo practice can't hire its way out and can't sell its way out.
The Polaris Pantheon is a third path: six AI agents that run entirely on clinic hardware, consolidate roughly 22 scattered projects into one coherent system, and turn evenings back into evenings. Built by a practicing family physician in Nebraska, in production today.

All six agents, STT, embeddings, and vector store run on clinic hardware via Ollama.
Ollama, whisper.cpp, Qdrant, and Postgres are software the practice runs — not services receiving PHI.
Marginal cost per encounter ≈ electricity. Cloud scribes run $200–600/month per provider.
Engine-adapter pattern: STT, LLM, and TTS are swappable by environment variable.
Clinical documentation and fax triage continue during ISP outages. The chart doesn’t.
Every design doc mirrors to a local git repo. The system outlives any cloud provider.
